Instructions to Access In & Out Urgent Care’s Results Portal

IMPORTANT***This is best done on a desktop computer***

You have had a lab test performed today that will be sent to an outside facility.
Results may take up to 1 week to return. If your results are NORMAL, they will be
SENT DIRECTLY VIA OUR ELECTRONIC PATIENT PORTAL as long as we
have a current email address on file. If there is ANYTHING about your results that
needs to be discussed directly with you, you will receive a PHONE CALL from our
clinic. Please call us in 7 days if you have not received results via phone or if you are
unable to access the portal. At that time, we can assist you in obtaining your results.

1. If you have previously set up an online portal with us, wait for an email from
no-reply@healthdataexchange.com indicating that you have results available or go
directly to the portal at web.healthdataexchange.com. Check your spam/junk mail if you are not seeing it in your email. Log in to your account with your existing
username and password. Move to step 3 below.*If you have NEVER SET UP an online portal with us, you will receive an email from no-reply@healthdataexchange.com asking you to create your secure patient portal. You will need to click on the link at the bottom that says “Click here” to verify your new account.

2. Register your account. The username will default to your email address, or you may change it if you would like. Please enter your birth date (or the patient’s birth date if you are the parent/guardian of a minor) and choose a password. Then hit “Create” at the bottom.

3. This should take you to a screen that says “My Account.” At the top left on the
banner next to My Account, there will be the word “Inbox” – click on “Inbox”. This
will take you to your results list.

4. Click on the line with the results you are wanting to open. There will be a paperclip icon to show there is an attachment. This will expand the results below it and will show a DOWNLOAD link. Click on the word download. This will pull up your
results in a viewable and printable form. Again, this is best viewed and printed from a desktop computer if you have one.

5. Please call the clinic where you were seen if you have any issues accessing the
portal or to reset your account, or you are always welcome to come in to pick up a
paper copy of your results.
